SNP Overview

SNPs target three different populations:

  • Institutionalized people.
  • People who are eligible for both Medicaid and Medicare.
  • People who have severe or disabling chronic conditions, as defined by the Centers for Medicare and Medicaid Services (CMS).

An SNP names the people who can qualify. The plan itself can be any insurance plan, like a Health Maintenance Organization (HMO) or Preferred Provider Organization (PPO). Let’s talk more about C-SNPs.

C-SNP Details

While a C-SNP is a Medicare Advantage Plan for people with chronic conditions, only certain conditions qualify. Typically, they must be severe or medically complex.

There are 15 conditions/categories:

  • Chronic Alcohol or Drug Dependence
  • Autoimmune Disorders limited to Polyarteritis nodosa, Polymyalgia, Polymyositis, Rheumatoid arthritis, and Systemic lupus erythematosus
  • Cancer — excluding pre-cancer conditions or in-situ status
  • Cardiovascular disorders restricted to Cardiac arrhythmias, Coronary artery disease, Peripheral vascular disease, Chronic venous thromboembolic disorder
  • Chronic Heart Failure
  • Dementia
  • Diabetes Mellitus
  • End-Stage Liver Disease
  • End-Stage Renal Disease requiring dialysis
  • Severe hematologic disorders like Aplastic anemia, Hemophilia, Immune thrombocytopenic purpura, Myelodysplastic syndrome, and Sickle-cell disease (excluding sickle-cell trait)
  • HIV/AIDS
  • Chronic Lung Disorders such as asthma, chronic bronchitis, emphysema, pulmonary fibrosis, pulmonary hypertension
  • Chronic and disabling mental health conditions like Bipolar disorders, Major depressive disorders, Paranoid disorder, Schizophrenia, and Schizoaffective disorder
  • Neurologic Disorder, including Amyotrophic lateral sclerosis (ALS), Epilepsy, Extensive paralysis (i.e., hemiplegia, quadriplegia, paraplegia, monoplegia), Huntington’s disease, Multiple sclerosis, Parkinson’s disease, Polyneuropathy, Spinal stenosis, and Stroke-related neurologic deficit
  • Stroke
