SNP Overview
SNPs target three different populations:
- Institutionalized people.
- People eligible for both Medicaid and Medicare.
- People with severe or disabling chronic conditions, as defined by the Centers for Medicare and Medicaid Services (CMS).
An SNP names the people who can qualify. The plan itself can be any insurance plan, like a Health Maintenance Organization (HMO) or Preferred Provider Organization (PPO). Let’s talk more about C-SNPs.
Qualifying for a D-SNP
People who are eligible for a D-SNP are people who qualify for both Medicare and Medicaid. To be eligible for Medicare, you have to belong to one of the three following populations: people 65 and over, younger people with End-Stage Renal Disease, and younger people with disabilities. Medicaid, on the other hand, is income-driven. You have to be low-income according to your state’s standards to qualify for Medicaid. If you’re eligible for both Medicare and Medicaid, you’re eligible for a D-SNP.
Benefits of a D-SNP
The main benefits of a D-SNP are the decreased out-of-pocket costs. Both the state and the federal governments help pay for a D-SNP patient’s healthcare. Some D-SNPs include prescription drug coverage, reducing those out-of-pocket expenses. There are some additional benefits, like:
- Meals
- a fitness benefit
- counter products
- smoking cessation
- hearing aids and glasses
- non-emergency transportation
Lastly, D-SNPs facilitate better care by introducing an Interdisciplinary Care Team (ICT). The ICT works together to create an Individualized Care Plan for each patient based on their Health Risk Assessment. This pathway helps doctors keep better track of their patient’s health needs.
